I rarely exchange links anymore, mostly because it's not very helpful to search engine rankings for the effort to put into them. I don't think they are a bad thing, I am just very selective and only do maybe 5-10 on any given site, if that, for just a "link page".
Instead I focus on targeted or home page link exchanges. The sites I do this with are directly related to the page their link is on, and I limit it to only a handful so as not to have too many outgoing links on one page. For example, I did home page link exchanges for this page:

You'll note those listed on that page have my link (to that exact page) on their home page or page related to mine. Also note I do have a link page, but it's named "resources" and actually doesn't have anything but my other sites. All other link exchanges done on that site are page to page. That site also gets several hundred unique visitors a day of very targeted traffic from search engines and real links.
I also focus on more incoming links than anything else. Meaning my site is linked somewhere, and I didn't have to exchange to do it. This would be like signatures, blog comments (NOT spam, just your site where it's allowed), articles, and press releases.
